Calcium hydroxide alone will not raise your calcium, all it will do is maintain calcium levels. Calcium chloride increases your calcium without raising ph. I do not use calcium chloride exclusively so I am not worried about the ionic imbalance. There is no formula, you have to test your water before adding anything but in time, you will get use to a routine.
Here is my regimen and the amount will vary from system to system. At lights out I drip calcium hydroxide without vinegar to counter the ph swing. When lights are on, I use kalkwasser with vinegar to avoid raising the ph of the water. When I do a water change the salt I use is Instant Ocean, the calcium and dkh levels in Instant Ocean are below the recommended levels, 450ppm calcium and 11 dkh for reefs. I add a DIY 2 part additive and some magnesium to 1350ppm.
